Please ensure that you have filled out all of the information as accurately as possible.  This will save us time in preparing our treatment and is particularly important to obtain approval for Bravo! FACT or Video FACT funding.  Once you have completed the information, please send Post DNA Productions the following ;

1. Lyric sheet(s) for the proposed song.  Please have typed on an 8 1/2" X 11" sheet or uploaded as a .txt or .doc file (See UPLOAD Below).
2. A copy of any Promotional Material (Press Kit, etc.) available for the Artist including photographs. You may also upload files (.txt, .doc, or Illustrator, etc.) containing this material (See UPLOAD Below).
3. A copy of the song(s) proposed on CD or cassette or upload .mp3(s) (See UPLOAD Below).
4. Copies of most recent previous music video (if any) on DVD / VHS or upload (See Below) any standard media files (Windows Media, Quicktime, Real Media or MPEG)
